To nominate an individual for one of the Guilford Green Foundation awards listed below, please complete this form and submit it and accompanying documentation to the GGF offices by March 1, 2020 . Any nominee not chosen for an award is automatically considered for the award by the next review committee. All individuals, LGBTQ and allies, are eligible for this award.

- GGF Visionary Award: The GGF Visionary Award is the most prestigious award given by the Guilford Green Foundation. It is given annually to an individual who has championed the vision of a better world for the LGBTQ Community.
- Dawn S. Chaney Award: The Dawn S. Chaney award is given to a woman that embodies strong leadership, advocacy and dedication to the LGBTQ community.
- Distinguished Service Award: The Distinguished Service Award is given to an individual who has provided years of outstanding service to the local LGBTQ community.
- Distinguished Leadership Award: The Distinguished Leadership Award is given to an individual who has demonstrated outstanding leadership for a specific event, project, or situation.
